Job summary

Frontpoint Health is seeking a Home Health RN to provide skilled nursing visits in patients' homes in Richardson, TX and surrounding areas. The RN will manage care within the license scope, perform OASIS visits, supervise visits when needed, and coordinate with the interdisciplinary team, patients, families, and referring agencies.

The role requires priority on accurate documentation (visit notes within 24 hours, OASIS within 48 hours), patient education, and maintaining HIPAA confidentiality

Qualifications

• Current Texas RN license required.
• Minimum 2 years RN home health experience.
• CPR certification current.
• TB test current.
• Experience with EMR systems (Kinsser preferred).
• Valid Texas driver’s license and auto liability insurance.

Responsibilities

• Case management and supervised visits.
• Perform skilled and comprehensive patient assessment at initial admission.
• Assess and document patient status during visits (vital signs).
• Provide skilled nursing care per the plan of care (medication, wound care, healing progress).
• Educate patients on home health care procedures.
• Maintain clinical records (visit notes within 24 hours; OASIS within 48 hours).
• Maintain patient confidentiality under HIPAA.
• Provide comfort through palliative care.

Job Title: Home Health - Full-Time - Registered Nurse (RN)

Location: Richardson, TX 75080

Job Summary

Our Registered Nurse (RN) with have a variety of duties that will help the company thrive and manage continuity of care. This RN will see all visits assigned within the scope of practice of their license which includes OASIS visits, skilled nursing visits, and supervised visits. They will also participate in the compensated perception of new hires. Administer skilled nursing care for patients of all ages in their place of residence and to coordinate care with the interdisciplinary team, patients, families, and referring agency. Assumes the responsibility for, and participates in, the coordination of care.

Job Responsibilities

• Case management and supervised visits.


• Perform skilled and comprehensive assessment of the patient during the initial admission.


• Assess and chart observations of the patient’s condition at each visit (vital signs.


• Provide skilled nursing care as needed in accordance with the plan of care (administering medication, cleaning/dressing wounds, assessing the healing progress).


• Provide proper patient education for home health care procedures and strategies.


• Responsible for the overall clinical record maintenance to include visit note (must be completed within 24 hours), comprehensive assessments, including OASIS (must be completed within 48 hours), medication profile, orders/plan submitted in a timely manner, etc.


• Maintain confidentiality of patient PHI through HIPPA law.


• Keep the patient comfortable through palliative care

Qualifications

• Current Texas State license as a Registered Nurse.


• 2+ years experience as a RN in Home Health.


• Current CPR certification.


• Reliable transportation.
