Job summary

Blue Ash Health & Rehab, LLC is seeking a compassionate registered nurse to join our home health team. You will visit patients at home, perform assessments, monitor vital signs, and administer medications per physician orders.

The ideal candidate holds an accredited nursing degree (Diploma/ADN/BSN), a current RN license in Ohio, CPR certification, a valid driver’s license, and a flexible schedule to travel locally. Strong communication with patients, families and care teams is essential.

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing.
• Current state RN license.
• CPR certification.
• Compassionate, friendly demeanor.
• Excellent communication skills.
• Current driver’s license and reliable transportation.
• Ability to work a flexible schedule and travel locally.

Responsibilities

• Visit patients in their homes for one-on-one visits.
• Complete initial and ongoing assessments to identify physical, psychosocial, and environmental needs of home health patients.
• Assess patients’ conditions, chart observations, and perform vital signs and medication reviews.
• Administer medications as prescribed by the patient’s physician.
• Educate patients and families on home health care strategies and procedures; suggest devices or tools to improve quality of life.
• Provide effective communication to patients, families, team members, and other health care professionals.
