Job summary

Buckeye Home Health Care LLC is seeking a Home Health RN to deliver patient-centered care in the comfort of patients' homes in the Dayton area. You will collaborate with physicians to manage medications, adjust plans, and prevent hospital readmissions, while supervising a team of LPNs, aides, and therapy staff.

Qualifications

• Active Ohio RN License (Required)
• Current CPR Certification (Required)
• Valid Auto Insurance (Required)
• Previous home health or related nursing experience preferred
• Strong communication, organization, and EMR skills
• Compassionate, patient-centered care mindset

Responsibilities

• Lead patient care: work directly with physicians to manage medications, adjust care plans, and help prevent readmissions.
• Coordinate the team: guide LPNs, aides, and therapy staff to deliver holistic care.
• Empower families: educate and counsel families on care processes and disease management.
• Seamless documentation: complete OASIS, recertifications, discharges, and ROCs in the EMR.
• Grow and connect: participate in orientation, staff meetings, and CEU opportunities.
