Job summary

CommuniCare Family of Companies seeks a dedicated Hospice RN Field Nurse to join Dignity Care Hospice in Blue Ash, OH. You will plan, organize, and direct home care services with emphasis on community health education and patient independence in home settings.

Requirements include an accredited nursing program graduate, active RN license, 1–2 years of acute care experience, and a bachelor’s degree with one year of home health care preferred. A valid driver’s license is required.

Qualifications

• Graduate of an accredited school of nursing.
• Active RN licensure in the relevant state.
• 1–2 years of acute care experience; BA with home health care experience preferred.

Responsibilities

• Complete initial patient and family assessments to determine home care needs.
• Perform comprehensive nursing assessments and document histories.
• Supervise and direct LPN and Home Health Aide in implementing the care plan.
• Develop and adjust patient education plans and implement them with the care team.
• Create and modify Home Health Aide care plans and monitor via feedback.
• Coordinate and communicate with physicians regarding patient needs and discharge plans.
