Job summary

Blue Ash Health & Rehab, LLC is seeking a compassionate LPN to join our growing home care team in Cincinnati, OH. The right candidate has a can-do attitude, excellent interpersonal skills, and a current state LPN license.

You will provide medical support to clients, assist with activities of daily living, and collaborate with RNs and physicians to follow the plan of care while maintaining a safe environment and using universal precautions.

Qualifications

• 1 year experience as an LPN in home care.
• Current state LPN license.
• Graduate of an accredited practical nursing program.
• Proof of CPR certification.
• Ability to work a flexible schedule and travel locally.
• Excellent interpersonal skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Participate in planning and coordination of total patient care with RN and physician’s plan.
• Accurately report and record patient condition and care.
• Prepare clinical/progress notes and summaries.
• Assist patient with activities of daily living and teach self-care techniques.
• Provide and maintain a safe environment for the patient.
• Assist physician and RN in complex nursing situations.
• Follow universal precautions during patient care.
• Follow emergency procedures in emergencies or significant patient condition changes.
