National Church Residences is seeking a Home Health RN Clinical Operations Coordinator to serve as the operational backbone of our home health agency, Monday–Friday 8am–5pm with flexible scheduling and mileage reimbursement for travel to Central and Southern locations.

You’ll oversee care plans, ensure regulatory compliance, mentor field staff, supervise scheduling, and manage daily workflows in our EMR (HCHB), collaborating with an interdisciplinary team to deliver high-quality patient care.

Competenze

• Active RN license in the state of practice.
• Minimum 2 years of home health leadership or clinical operations experience.
• Strong knowledge of home health regulations and reimbursement.
• Proficiency in Microsoft Office; ability to learn new EMR systems quickly.
• Excellent communication, organizational, and problem-solving skills.
• Valid driver’s license and reliable transportation (up to 25% travel).
• Home health certification (CHPN, CCM) preferred.

As a Clinical Operations Coordinator, you'll be the operational backbone of our home health agency — ensuring quality patient care, regulatory compliance, and smooth daily workflows.

What You Bring:
