The role requires an active RN license in the state, 2+ years in home health leadership, strong communication and problem-solving skills, and a willingness to travel up to 25%.

Qualifications

• Active RN license in state of practice.
• 2+ years in home health leadership or clinical operations.
• Strong knowledge of home health regulations and CPs.
• Proficiency with MS Office; able to learn EMR systems quickly.
• Excellent communication and organizational skills.
• Valid driver’s license; reliable transportation; up to 25% travel.
• Home health certification (CHPN, CCM) preferred.

# **Job Description:**# Home Health RN Clinical Operations Coordinator**Monday–Friday | 8am–5pm | Flexible Schedule | Mileage Reimbursement** *Supporting Central & Southern locations*## ## About the RoleAs a Clinical Operations Coordinator, you'll be the operational backbone of our home health agency — ensuring quality patient care, regulatory compliance, and smooth daily workflows.
