Job summary

National Church Residences is seeking a Home Health RN Clinical Operations Coordinator to serve as the operational backbone of our home health agency, ensuring quality patient care, regulatory compliance, and smooth daily workflows across multiple locations.

You will lead an interdisciplinary team, approve care plans, supervise nursing staff, oversee admissions and recertifications, and manage daily workflows in HCHB. Travel up to 25% is required.

Qualifications

• Active RN license in state of practice.
• 2+ years of home health leadership or clinical operations experience.
• Strong knowledge of home health regulations, reimbursement, and conditions of participation.
• Proficiency in Microsoft Office; ability to learn new EMR systems quickly.
• Excellent communication, organizational, and problem-solving skills.
• Valid driver’s license & reliable transportation (up to 25% travel).
• Home health certification (CHPN, CCM, or equivalent) preferred.

Responsibilities

About the Role

As a Clinical Operations Coordinator, you’ll be the operational backbone of our home health agency — ensuring quality patient care, regulatory compliance, and smooth daily workflows. You’ll work alongside an interdisciplinary team while supporting and mentoring field staff across multiple locations.

What You’ll Do:
