National Church Residences seeks an experienced Home Health RN Clinical Operations Coordinator to serve as the operational backbone of the home health agency, ensuring quality patient care and compliance. You will mentor field staff, manage daily workflow, and oversee EMR documentation across multiple locations.

The role emphasizes collaboration with an interdisciplinary team, audits for COP compliance, and on-call coverage to sustain continuous clinical support. Travel up to 25% may be required.

Qualifications

• Active RN license in the state of practice.
• 2+ years in home health leadership or clinical operations.
• Strong knowledge of home health regulations, reimbursement, and CoP.
• Proficiency in Microsoft Office and ability to learn EMR systems quickly.
• Excellent communication, organizational and problem-solving skills.
• Valid driver's license and reliable transportation (up to 25% travel).
• Home health certification (CHPN, CCM, or equivalent) preferred.

Responsabilités

• Provide clinical oversight by approving care plans and monitoring outcomes.
• Conduct audits and ensure compliance with home health regulations and CoP.
• Train, mentor, and supervise nursing staff; support onboarding and scheduling.
• Oversee admissions, recertifications, and resource scheduling.
• Manage daily workflows and documentation in HCHB EMR.
• Participate in on-call rotations to provide continuous clinical support.
