Job summary

Granite Peak Home Health is seeking an experienced Clinical Manager (RN) to lead patient-care in Montana. You will supervise clinical staff, coordinate care, and ensure compliant, high-quality home health services in your region.

Responsibilities include directing assessments, staffing, budgets, policy development, and ongoing staff training, with emphasis on CMS/PDGM guidelines and regulatory standards. A Montana RN license and Oasis-E proficiency are required for this role.

Qualifications

• Bachelor’s degree in Nursing required.
• Current Montana RN license.
• Minimum 2 years nursing experience in Home Health preferred.
• Proficiency with OASIS-E assessment.
• Proficiency with WellSky and web-based systems.
• Must pass background check and not be listed on the OIG exclusions list.

Responsibilities

• Oversee all home health patient care in the designated region.
• Provide direct supervision of assessment, planning, implementation and evaluation of patient and caregiver care.
• Lead, manage, and hold staff accountable for quality and timely care delivery.
• Schedule and conduct weekly IDT and leadership meetings per CMS and PDGM guidelines.
• Develop and implement processes, policies, and procedures for patient care services.
• Assist in preparing and administering the agency budget.
• Monitor census trends and adjust staffing, revenue, and expenses accordingly.
• Ensure adherence to company policies and regulatory standards.
• Participate in hiring, evaluating, and terminating personnel.
• Oversee onboarding, in-service training, and continuing education programs.
• Conduct quarterly check-ins and annual performance reviews.
• Ensure accurate patient documentation and reporting.
• Maintain adequate clinical supplies and equipment.
• Support QAPI initiatives to ensure safe, high-quality patient care.
