Job summary

Tufts Medicine Home Health is seeking a registered nurse to coordinate and deliver professional home health care. You will triage calls, plan and evaluate patient care, and work with the clinical management team to ensure timely, compliant service delivery.

The role requires active RN licensure in the service state, 3 years of community health experience or a combination with home health, and CPR/BLS certification as specified. 40-hour weeks in Lowell, MA with full-time responsibilities.

Qualifications

• Must hold an active RN license in the state where services are provided.
• Three (3) years of community health experience required or a combination of at least one year of acute medical/surgical nursing experience and 2 years of home health experience.
• CPR Certification required upon hire if hired before 9/28/2026; if hired after, BLS must be obtained within 30 days.

Responsibilities

• Triages calls from physician and follows through on required actions with field staff.
• Coordinates clinical services in collaboration with clinical staff to deliver timely patient care.
• Communicates with Clinical Managers and Clinical Director on practice issues and performance.
• Ensures compliance with regulatory guidelines and agency policies.
• Provides orientation and preceptorship to clinical staff.
• Evaluates staff performance and supports competency development.
