Job summary

Tufts Medicine Care at Home is looking for an RN Clinical Coordinator in Lowell, MA to support the clinical management team in delivering home-based care. The role triages patient care calls, coordinates staff coverage, and ensures timely, compliant delivery of nursing services according to physician orders.

The ideal candidate holds an RN license with 3 years of community health or related home health experience, and prior telephone triage experience is a plus.

Qualifications

• Registered Nurse (RN) license required.
• Three years of community health experience, or mix of acute and home health experience.
• Preferred: telephone triage experience and familiarity with Medicare participation.

Responsibilities

• Coordinate clinical services with the care management team.
• Triages calls and caregiver scheduling related to patient care.
• Collaborate with clinical managers and director on practice issues.
• Document care plans, ensure reimbursement, and maintain HIPAA compliance.
• Provide guidance on visit frequency, patient status, and care interventions.
