Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager in Ames, IA. You will coordinate all aspects of the patient’s home care, perform admission visits, determine eligibility for home health services, and develop the plan of care to be followed.

Strong clinical skills and an active RN license are required. Requirements include a current RN license, a valid driver’s license, CPR certification, and 1 year of RN experience (2 preferred).

Qualifications

• Graduate of an accredited nursing program (Diploma/Associate/Bachelor).
• Current state RN license required.
• Valid driver’s license and reliable transportation.
• Minimum of 1 year RN experience; 2 years preferred.
• CPR certification; Hepatitis consent/declination.
