Job summary

Angels Care Home Health is seeking a Registered Nurse Case Manager to coordinate patient home care. You will assess, plan, and implement care in collaboration with the medical team, and perform admission visits to set up services.

Qualifications

• Graduate of an accredited nursing program (diploma/associate/bachelor).
• Current state RN license.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care as RN Case Manager.
• Perform admission visit to patient’s home and assess eligibility for home care.
• Develop and follow the plan of care with interdisciplinary team.
