Job summary

Angels Care Home Health is seeking a Registered Nurse Case Manager to coordinate comprehensive home care for patients in the community. You will conduct admission visits, assess eligibility for home care, and craft individualized plans of care in collaboration with the interdisciplinary team.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of CPR and hepatitis consent/declination.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed.
• Perform admission visits to patients in the home; determine eligibility for home care services.
• Develop and implement the plan of care to be followed.
