Qualifications

• Graduate from an accredited school of nursing.
• Current Illinois RN license with CPR certification.
• One year of medical-surgical experience preferred.
• Excellent communication, documentation, and computer skills.
• Ability to work with multidisciplinary teams and families.

Responsibilities

• Develop and maintain therapeutic nursing relationships across care continuum.
• Coordinate care and discharge planning with interprofessional team.
• Assess patient readiness and involve family in care planning.
• Ensure quality, cost-effective care and adherence to regulatory requirements.
