Job summary

Qualifications

• Minimum of 1 year of RN experience in a clinical setting; home health experience preferred.

• Ability to perform accurate assessments, develop plans of care, and document thoroughly.

Responsibilities

• Oversees the overall care of the patient from beginning to end of episode.
• Updates orders and coordinates multidisciplinary care per physician direction.
• Performs OASIS assessments to develop an individualized plan of care.
• Educates patients and families about disease processes and care plans.
• Monitors patient conditions and communicates changes to the care team.
• Ensures accurate documentation and compliance with regulations.
