Job summary

The Christ Hospital Home Health Care in Cincinnati is seeking an On-Call Registered Nurse to support a team of nurses and patients after hours. You will provide one-on-one home care, administer meds, and work at the top of your license within a multidisciplinary team.

Responsibilities include developing individualized plans, evaluating treatments, and ensuring timely documentation and regulatory compliance. A valid Ohio RN license is required, with remote work when no visits are needed.

Qualifications

• Minimum 1 year RN experience in acute care.
• Home health/home care experience preferred.
• Ability to work after-hours and manage schedules.

Responsibilities

• Respond to after-hours patient needs and provide one-on-one care.
• Administer medications as prescribed by the physician.
• Coordinate with the interdisciplinary team for optimal care.
• Complete SOC/ROC/Recert and chart reviews within 24 hours.
• Educate patient and family while documenting care and outcomes.
