Job summary

The Christ Hospital Home Health Care is seeking an On-Call Registered Nurse in Cincinnati, OH to support after-hours patient needs and provide one-on-one care in patients' homes. You may review charts and respond to calls from home when after-hours demand is low.

You will work with an interdisciplinary team to ensure high-quality care, perform OASIS as needed, and document care in accordance with Medicare guidelines. A valid RN license in Ohio is required.

Qualifications

• Active RN license in the state of employment.
• Minimum one year RN experience in an acute care setting; home health experience preferred.
• Excellent communication and patient care skills.

Responsibilities

• Observe and monitor patient conditions during home visits and respond to after-hours needs.
• Provide skilled nursing care as directed by the physician and document in chart notes.
• Educate patients and families on disease processes and care plans using teach-back methods.
