Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of home care and interact with other medical disciplines as needed. You will conduct admission visits to assess eligibility and develop personalized care plans for patients in the Home Health setting.

Requirements include an RN license, current CPR, a valid driver’s license, and reliable transportation with auto liability insurance; one year of RN experience is preferred.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Perform the admission visit to determine eligibility for home care services and develop the plan of care.
• Interact with other medical disciplines as needed to provide professional care.
