Job summary

Angels Care Home Health is seeking an RN Case Manager to coordinate all aspects of patient home care. You will perform admission visits, assess eligibility for home health services, and develop the plan of care in collaboration with the care team.

The ideal candidate is a licensed RN with 1 year of bedside experience and a drive to grow in a home health setting. A valid driver's license and reliable transportation are required.

Qualifications

• Graduate of an accredited nursing program (diploma, ADN or BSN).
• Current state license as a Registered Nurse (RN).
• One year RN experience; two years preferred for case management.
• Proof of CPR and Hepatitis consent/declination.
• Valid driver's license and reliable transportation.
• Auto liability insurance is required.

Responsibilities

• Coordinate all aspects of the patient’s home care as the RN Case Manager.
• Perform admission visits to determine eligibility for home care services.
• Develop and implement the plan of care in collaboration with the patient and care team.
