Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of patient home care. You will perform admission visits in the home, determine eligibility for home care services and develop the plan of care in collaboration with the patient and family.

The role requires an active RN license, a valid driver’s license, reliable transportation, and at least one year of RN experience (two years preferred).

Qualifications

• Graduate of an accredited nursing program.
• Current state RN license.
• Valid driver’s license required.
• One year of RN experience preferred.

Responsibilities

• Coordinate all aspects of the patient’s home care; act as liaison with medical teams.
• Conduct admission visits in the patient’s home and determine eligibility for home care services; develop the plan of care.
