Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate home health care for patients. You will interact with other medical disciplines, perform admission visits, assess eligibility, and develop a comprehensive care plan to be followed in the patient’s home.

Ideal candidates have an accredited nursing degree, an active RN license, a valid driver’s license, reliable transportation, CPR certification, and at least one year of RN experience (two preferred).

Qualifications

• Graduate of an accredited diploma, associate or baccalaureate nursing program.
• Current state RN license is required.
• Current state driver's license is required.
• One year RN experience; two years preferred.
• CPR certificate and Hepatitis consent/declination documentation.
• Reliable transportation with auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care as an RN Case Manager.
• Perform admission visit and determine eligibility for home care services.
• Develop and follow the patient’s plan of care.
• Collaborate with other medical disciplines as needed.
