Job summary

Angels Care Home Health is seeking an experienced Registered Nurse (RN) Case Manager to coordinate all aspects of in-home patient care. You will interact with other medical disciplines as needed and provide professional nursing care to patients in their homes.

You will also perform admission visits to assess eligibility for home health services and develop the plan of care to be followed by the care team. This role requires an active RN license and a valid driver’s license, with a strong

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• The RN Case Manager coordinates all aspects of the patient’s home care.
• You will interact with other medical disciplines as needed and provide professional care to the patient.
• You will perform the admission visit to the patient in the home and determine eligibility for home care services.
