Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate comprehensive home health services. You will conduct admission visits, assess eligibility, and develop individualized plans of care for patients in their homes, collaborating with multidisciplinary teams.

Qualifications
- Graduate of an accredited nursing program (diploma, associate, or baccalaureate).
- Current state RN license.
- CPR certification and Hepatitis consent/declination.
- Valid driver's license and reliable transportation.
- Auto liability insurance coverage.
Responsabilités
- Coordinate all aspects of the patient’s home care as an RN Case Manager.
- Interact with other medical disciplines as needed to provide professional care.
- Perform admission visit to determine eligibility for home care services.
- Develop and follow the plan of care for each patient.
