Job summary

Angels Care Home Health is seeking an Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. The role starts with an admission visit, assessing eligibility for home care, and developing a care plan in collaboration with the patient and care team.

You will work with multiple medical disciplines to provide high-quality care, ensuring timely plan updates and adherence to guidelines. A valid RN license, driver’s license, CPR, and hepatitis vaccination are required.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Valid driver’s license and reliable transportation.
• One year of RN experience; two years preferred.
• Proof of CPR and hepatitis vaccination/declination.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Perform admission visit to the home and determine eligibility for home care services.
• Develop and follow the plan of care in collaboration with the patient and team.
• Interact with other medical disciplines as needed to provide professional care.
