Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of home-based patient care. You will conduct admission visits, assess eligibility for home care services, and develop individualized plans of care in collaboration with a multidisciplinary team.

Qualifications

• Graduate of an accredited diploma, ADN or BSN program.
• Current state RN license in good standing.
• Valid driver’s license.
• One year of RN experience preferred; two years preferred.
• CPR certification and hepatitis consent/declination.
• Reliable transportation with current auto insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed to provide care.
• Perform admission visit and determine eligibility for home care services.
• Develop and follow the plan of care for each patient.
