Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, assess needs, coordinate care, support discharge planning, and manage ongoing outpatient care under supervision. You will educate, coach, and assist the care team to ensure appropriate levels of care and outcomes.

The role involves collaboration with a Care Advocate and Social Worker, and requires strong clinical experience, CPR certification, and a valid RN license.

Qualifications

• RN license in the applicable state(s)
• CPR certification required
• Valid driver's license and reliable transportation
• 1+ year of clinical RN experience

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other disciplines as indicated by patient needs
• Promotes patient health and independence through education and self-care coaching
• Supervises LPNs and HHAs
• Completes documentation timely and accurately
• Communicates patient progress to the clinical manager and care team
• Plans and provides staff education
• Performs on-call responsibilities and services as assigned
• Participates in clinical development and continuing education programs
• Other duties as assigned
