Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, coordinating care and supporting discharge and transition planning under RN or MD supervision. You will ensure appropriate care levels, provide education, coaching, and decision support, and collaborate with Care Advocates and Social Workers.

Qualifications

• Current RN license, valid in the states you serve.
• Current CPR certification.
• 1+ years of RN clinical experience.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform patient assessments and collaborate to develop a care plan.
• Make referrals as needed and document rationale.
• Promote health and independence through teaching and rehab.
• Supervise LPNs and HHAs.
• Complete timely and accurate documentation.
• Communicate patient progress to clinical manager.
• Plan and provide staff education.
• Participate in on-call duties and services.
• Engage in clinical development and continuing education.
• Perform other duties as assigned.
