Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, coordinating care across discharge, transition planning and outpatient services. You’ll educate, coach and support treatment decisions while collaborating with a Care Advocate and Social Worker.

Primary responsibilities include conducting patient assessments, making referrals, and promoting self-care through teaching and rehabilitative measures, with on-call duties and ongoing

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• 1+ year of clinical experience as a Registered Nurse (RN).
• Current CPR certification; valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Assess patient needs and coordinate care with the care team.
• Support discharge and transition planning and ongoing outpatient care.
• Educate, coach, and provide treatment-decision support to patients and families.
• Supervise LPNs and HHAs and complete timely, accurate documentation.
• Communicate patient progress to the clinical manager and care team.
• Plan and provide staff education; participate in on-call responsibilities.
• Engage in clinical development and continuing education.
