Job summary

Optum is seeking a Nurse Case Manager to guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge planning, and managing ongoing outpatient care under supervision of an RN or MD.

Responsibilities include performing patient assessments, making referrals, teaching self-care, supervising LPNs and HHAs, timely documentation, and regular progress updates to the clinical team. Strong RN experience and licensure are required.

Qualifications

• Current RN license valid in the state(s) you are assigned to work.
• Valid driver's license, reliable transportation, and liability insurance.
• Current CPR certification is required.
• 1+ year of clinical experience as an RN.

Responsibilities

• Assess patient needs and coordinate a care plan with the care team.
• Assist discharge and transition planning and manage outpatient care.
• Provide education, coaching, and decision support; supervise LPNs and HHAs.
• Document timely and accurately; communicate patient progress to the clinical team.
