Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care under supervision of an RN or MD.

As a full-time role, you will collaborate with a Care Advocate and Social Worker, provide education and treatment-decision support, and participate in ongoing professional development within a supportive organization.

Qualifications

• Current RN license valid in the assigned state(s).
• Minimum 1 year of clinical RN experience.
• CPR certification required.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documented rationale.
• Promotes patient health and independence through teaching and appropriate rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
