Job summary

The Pennant Group, Inc. is seeking a compassionate RN Case Manager for the East Portland/PDX area to plan, direct, and coordinate home health services.

The RN will assess patients and families, develop care plans, and work with the interdisciplinary team to meet individual goals. Responsibilities include administering medications per plan, educating patients and families, and participating in on-call duties.

Qualifications

• Graduate of an accredited school of nursing.
• One (1) to two (2) years of acute care experience in institutional setting; home health experience preferred.
• Currently licensed as a Registered Nurse in the state(s) of planned practice.
• CPR certification.
• Excellent observation, verbal and written communication skills; problem solving and basic math skills.
• Self-directed and able to work with minimal supervision.

Responsibilities

• Assesses patient and family needs; develops an initial plan of care with the certifying medical provider.
• Provides comprehensive nursing care and administers medications per plan of care.
• Acts as Case Manager coordinating patient care with Clinical Manager and interdisciplinary team.
• Documents patient services and status timely and accurately; communicates changes to the medical provider.
• Participates in on-call duties and care coordination activities.
