Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, assess needs, coordinate care, support discharge and transition planning, and manage ongoing outpatient care under supervision.

You will collaborate with a Care Advocate and a Social Worker, perform patient assessments, referrals, teaching, and supervise LPNs and HHAs, with a focus on quality, safety, and patient-centered outcomes in a home-health setting.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• 1+ years of clinical experience as a Registered Nurse (RN).
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale.
• Promotes health and independence through teaching and rehabilitative measures; teaches self-care.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call duties and services as assigned.
• Participates in clinical development and continuing education.
• Other duties as assigned.
