Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, coordinating care and supporting discharge and transition planning. Under supervision of an RN or MD, you will ensure appropriate care levels and provide education, coaching, and decision support.

You will collaborate with a Care Advocate and Social Worker, perform patient assessments, supervise staff, and participate in ongoing clinical development.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• 1+ year of clinical experience as a Registered Nurse (RN).
• Valid driver's license, access to reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Merges with other disciplines as needed based on patient needs.
• Promotes patient health and independence through teaching and appropriate rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
