Symbii Home Health & Hospice in Preston, Idaho, is seeking a registered nurse to provide skilled in-home care. The role emphasizes comprehensive patient assessments, care planning, and coordination with families and physicians.

The ideal candidate holds an Idaho RN license, has a valid driver’s license, and preferably experience in home health and hospice settings. On-call duties and patient-centered care are integral parts of this position.

Qualifications

Idaho RN license is required.
Valid driver’s license is required.
Home Health and Hospice experience preferred.

Responsabilités

Completes an initial assessment of patient and family to determine home care needs.
Regularly re-evaluates patient nursing needs.
Initiates the plan of care and revises as patient status changes.
Uses health data to determine nursing diagnosis.
Develops a care plan with goals and family involvement.
Administers medications and treatments as prescribed.
Counsels patient and family on nursing needs.
Provides health instruction per plan of care.
Identifies discharge planning needs and coordinates prior to discharge.
Acts as Case Manager for assigned caseload.
Maintains documentation and standards of conduct.
Prepares clinical notes and updates primary physician at least every 60 days.
Communicates patient needs to physician and coordinates care with community health professionals.
Participates in on-call duties and ensures equipment and services are available.
