Job summary

Urrly Hospice is seeking a dedicated RN Case Manager in Twin Falls, Idaho to guide patients and families through compassionate end-of-life care. This field-based role covers a defined service area with strong team support and comprehensive benefits.

You will perform home visits, assess symptoms, coordinate care, and document in CareEfficient EMR, while collaborating with the interdisciplinary team. Idaho RN license or compact licensure is required plus at least 1 year of post-graduate RN

Qualifications

• Current Idaho RN license or compact license or clear path to Idaho licensure.
• At least 1 year of post-graduation RN experience; new grads not considered.
• Hospice, palliative, home health or other clinically intensive RN background.
• Ability to work independently in the field with close care-team collaboration.
• Strong communication with patients, families, physicians, facilities and teammates.

Responsibilities

• Manage hospice patient caseload across Twin Falls/Magic Valley.
• Complete skilled nursing visits in patients' homes and care settings.
• Assess symptoms, coordinate medications and care plan needs, and adapt quickly.
• Educate and support patients, families, caregivers, and facilities.
• Collaborate with interdisciplinary team to deliver patient-centered care.
• Document care accurately in CareEfficient or similar EMR.
