Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, coordinating services from admission through discharge planning and ongoing outpatient care.

Under supervision of an RN or MD, you will help ensure appropriate care levels, provide education, coaching, and treatment-decision support while collaborating with a Care Advocate and Social Worker.

Qualifications

• Current RN license in state(s) assigned to work in
• Current CPR certification
• 1+ year of clinical experience as an RN
• Valid driver's license with reliable transportation

Responsibilities

• Performs patient assessments with care team to develop a care plan
• Makes referrals to other disciplines as needed
• Educates patients and families on self-care techniques
• Supervises LPNs and HHAs
• Completes documentation timely and accurately
• Communicates patient progress to clinical manager and care team
• Participates in staff education and on-call responsibilities
• Engages in clinical development and continuing education programs
• Other duties as assigned
