Job summary

Bridge Home Health & Hospice is seeking a Registered Nurse to plan, organize and direct home care services in the Central San Diego area. The role emphasizes community health education, assessment and collaboration with families within their homes and communities.

The RN will perform initial and ongoing patient assessments, develop and revise care plans, administer prescribed treatments, and coordinate with physicians and health professionals. Strong communication and driving requirements apply.

Qualifications

• Graduate of an accredited school of nursing.
• Minimum of 2 years licensed experience (California).
• Current licensure in state and CPR certification.
• Bachelor’s degree, with one year home health care experience preferred.
• Management experience not required.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and revises as patient status changes.
• Develops a care plan with goals based on nursing diagnosis and family involvement.
• Administers medications and treatments as prescribed by the physician.
• Acts as Case Manager and coordinates patient care for assigned caseload.
• Prepares clinical notes and updates the primary physician at least every 60 days.
• Communicates with physician and community health professionals to coordinate care.
• Participates in on-call duties and ensures resources for care are available.
