Job summary

Amedisys, a leading home health and hospice provider, is hiring a Registered Nurse case manager for full-time day shifts. You will assess patients, collaborate with the care team, and develop care plans to support healing at home.

You will supervise LPNs and HHAs, ensure timely documentation, and keep the clinical manager informed of patient progress. Required: 1+ year RN experience, active RN license, CPR, and a valid driver’s license.

Qualifications

• 1+ year of clinical experience as an RN.
• Current RN license in applicable state(s).
• CPR certification required.
• Valid driver's license with reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Provides staff education and participates in on-call coverage.
