Job summary

Amedisys is hiring a Registered Nurse Case Manager to assess patients, coordinate plans of care, and supervise LPNs and HHAs in a home-health setting. You will document care plans and communicate progress to the clinical team, ensuring timely and accurate records while supporting patient independence at home.

Eligible candidates have 1+ year RN experience, a current state RN license, CPR certification, and a valid driver’s license. Mileage reimbursement and benefits are included where applicable.

Qualifications

• RN with 1+ year clinical experience required.
• Current state RN license and CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans staff education and participates in on-call responsibilities.
• Participates in clinical development and continuing education.
