Qualifications

• One (1) to two (2) years of recent acute care experience in an institutional setting.
• Must be a licensed driver with an insured and well-maintained automobile.

Responsibilities

• Completing an initial patient assessment to determine home care needs.
• Supervising and directing patient care by LPN/Home Health Aide.
• Developing and modifying patient education and care plans.
• Administering medications and treatments as prescribed.
• Coordinating patient care as Case Manager.
