Alternate Solutions Health Network is seeking a Registered Nurse Case Manager to lead patient assessment, plan of care, and coordination across disciplines in the home health setting. You will educate patients and families, document accurately, and ensure compliance with regulations.

Qualifications

• Registered Nurse with current license in the state of employment.
• Home health experience is desired.
• Strong ability to coordinate care across disciplines.

Responsabilités

• Oversees the overall care of the patient from beginning to end of episode.
• Updates all doctor orders in patient chart accordingly.
• Notifies doctor of any significant changes in patient condition.
• Manages multi-disciplinary care and provides skilled nursing care at home.
• Performs OASIS assessments to develop an individualized plan of care.
• Administers medication as prescribed by the physician.
• Educates patients and families about disease processes and care plan.
