Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will work with various medical disciplines, perform admission visits, determine eligibility for home care services, and develop the plan of care to be followed.

Qualifications include a nursing degree from an accredited program, a current RN license, a valid driver’s license, CPR/Hepatitis documentation, and reliable transportation.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed.
• Perform the admission visit to the patient in the home.
• Determine the patient’s eligibility for home care services.
• Develop the plan of care to be followed.
