Job summary

Advocate Health Care is seeking a Registered Nurse (RN) Case Manager for Home Health in Illinois. The role involves building therapeutic nurse-patient relationships, coordinating care across the healthcare continuum, and ensuring high-quality, cost-effective care with on-call rotation.

Scheduled as full-time, Monday–Friday 8:00 AM–4:30 PM with weekend and on-call rotations; includes a $3,000 sign-on bonus for external candidates. Must hold Illinois RN license and CPR certification.

Qualifications

• Graduate from an accredited school of nursing.
• One year of medical surgical experience.
• Current Illinois RN license.
• Current CPR.

Responsibilities

• Nurse/Patient Relationship: Develops and maintains a therapeutic relationship throughout the healthcare continuum.
• Assuring/Improving Quality Care: Facilitates the patient and family’s right to receive quality, cost effective care and mobilizes resources as needed.
• Patient/Staff Education: Provides education to patients, families, and nursing peers to promote positive outcomes.
• Nursing Process: Applies nursing process to assess, plan, implement, and evaluate care for patients and coordinate with the interdisciplinary team.
