Qualifications

• Graduate of an accredited school of nursing.
• Minimum of 2 years licensed experience (California).
• Current licensure in state and CPR certification.
• Bachelor's degree, with one (1) year of home health care experience preferred.
• Management experience not required. Responsible for supervising home health aides.
• Excellent observation, verbal and written communication skills, problem solving skills; nursing skills per competency checklist.
• Prolonged or considerable walking or standing. Able to lift, position or transfer patients; lift supplies and equipment.
• Must possess a valid California Driver's License with automobile in good working order and insured.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes revisions as patient status changes.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis and coordinates family involvement.
• Initiates preventive and rehabilitative nursing procedures; administers physician-prescribed medications.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to patient as appropriate.
• Identifies discharge planning needs and implements prior to discharge.
• Acts as Case Manager and coordinates patient care for assigned caseload.
• Prepares clinical notes and updates the primary physician at least every 60 days.
• Communicates with physician regarding patient needs and changes; obtains physician orders.
• Coordinates care with community health-related persons.
• Participates in on-call duties as defined by policy.
• Ensures arrangements for equipment and services are available.
• Instructs, supervises, and evaluates home health aide care every two weeks.
• Adheres to attendance and reliability guidelines to ensure quality patient care.
