Job summary

Sunrise Home Health Services is seeking a compassionate Home Health Registered Nurse (RN) Case Manager to join our team serving South Central Phoenix and surrounding communities. You will manage a dedicated territory with a manageable caseload, providing hands-on nursing care and clinical case management in patients' homes.

The role offers full-time (and PRN) opportunities, with a supportive clinical team and autonomy to coordinate care while ensuring high-quality patient outcomes.

Qualifications

• Current Arizona RN license or Compact license.
• Graduate of an accredited nursing program.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Manage an assigned caseload within the South Central Phoenix/Northwest Phoenix Valley area.
• Complete SOC, ROC, Recertification, and Discharge assessments.
• Develop individualized plans of care based on patient needs.
• Provide skilled nursing care, wound care, and monitor treatment effectiveness.
• Administer medications, reconcile prescriptions, and educate patients.
• Monitor patient condition and communicate with physicians and the care team.
• Educate patients and caregivers on disease management and safety.
• Coordinate care with therapists, social workers, and other professionals.
• Complete timely documentation in compliance with Medicare and agency requirements.
• Support patients to improve independence, safety, and quality of life at home.
