Job summary

Phoenix Home Care in Springfield, MO is seeking a Home Health Case Manager to coordinate in-home patient care, collaborate with nurses, therapists, and physicians, and develop personalized care plans aligned with orders and initial assessments.

You will oversee LPN visits, manage varied weekly caseloads, educate patients on CHF, COPD, diabetes, wound care and med management, and participate in charting with our AI-supported tools to support work-life balance.

Qualifications

• Missouri RN license or compact license required.
• Driver's license and ability to drive to patient homes.
• Home health or critical care experience preferred.

Responsibilities

• Manage caseload including supervision of LPN visits.
• Handle varied weekly caseload with visits, evaluations and start of care.
• Educate patients on CHF, COPD, diabetes, wound care and med management.
• Communicate with physicians to obtain orders and write care plans.
• Collaborate with PT/OT/Speech therapy teams.
