Qualifications
- Current Arizona RN license and graduation from an accredited nursing program.
- Strong clinical assessment, communication, and organizational skills.
- Ability to work independently in a home-based clinical environment while collaborating with an interdisciplinary team.
Responsibilities
- Manage an assigned caseload within the Southwest Phoenix Valley.
- Complete SOC, ROC, Recertification, and Discharge assessments.
- Provide skilled nursing care, wound care, and medication management.
- Coordinate care with therapists, physicians, and other health professionals.
- Document accurately per Medicare, regulatory, and agency requirements.
