Sunrise Home Health Services is seeking a compassionate Registered Nurse (RN) Case Manager to serve the Southwest Phoenix Valley, including Goodyear, Avondale, Litchfield Park, Glendale, and Laveen, delivering high-quality home health care at patients' homes.

Formación

• Current Arizona RN license and graduation from an accredited nursing program.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently in a home-based clinical environment while collaborating with an interdisciplinary team.

Responsabilidades

• Manage an assigned caseload within the Southwest Phoenix Valley.
• Complete SOC, ROC, Recertification, and Discharge assessments.
• Provide skilled nursing care, wound care, and medication management.
• Coordinate care with therapists, physicians, and other health professionals.
• Document accurately per Medicare, regulatory, and agency requirements.
