Job summary

Sunrise Home Health Services in Phoenix seeks a compassionate Registered Nurse Case Manager to join our team serving South Central Phoenix, including Phoenix, Glendale, Laveen, and Avondale. You will manage a dedicated territory with a manageable caseload and strong clinical leadership support.

Qualifications

• Current Arizona RN license or Compact license valid in Arizona.
• Graduate of accredited nursing program.
• CPR certification.
• Valid driver's license and transportation.
• Ability to work independently in home health and collaborate with a team.

Responsibilities

• Manage an assigned caseload in the South Central Phoenix area.
• Complete Start of Care, Recertifications, and Discharge assessments.
• Develop and revise individualized plans of care.
• Provide skilled nursing care and wound management as needed.
• Educate patients and caregivers on disease management and medications.
• Coordinate with therapists and other care professionals.
• Document accurately per Medicare and agency requirements.
• Aim to reduce unnecessary hospitalizations through proactive care.
